About work permits
As a community service, Operation Fresh Start serves as a Work Permit Office. Some of our staff members are registered as Work Permit Officers and can create work permits for youth under the age of 16 who have recently obtained a job.
The youth must be present in order to create the work permit.

At OFS, work permits are issued Monday through Thursday, 8:00 a.m. – 4:00 p.m. Please call ahead to confirm a staff member is available to assist you.

What do I need to bring to get a work permit?
The following describes the documents to bring with you to receive a work permit. Copies of documents will not be accepted.

Proof of age – either a birth certificate, Wisconsin driver’s license, Wisconsin State ID, or a baptismal certificate

Social Security Card

Letter from the minor’s employer stating the employer’s intention to hire the minor, describing the job duties and hours of work, and signed by the employer’s representative

Letter from the minor’s parent or guardian consenting to the employment. Alternatively, parents/guardians may sign the letter from the employer.

$10.00 to cover the work permit fee. We accept cash (exact change) or checks payable to Operation Fresh Start.
Operation Fresh Start will issue a receipt along with the work permit.
